The purpose of this letter is regarding cosponsorship of the program titled Chiro to EMT scheduled in Omaha, NE on September 24-28, 2002.

Our co-sponsor administrative fee is $500 per year with a $30 per attendee fee for each seminar. The only additional fees that you would incur would be license renewal application fees and postage.

It is our understanding that you are responsible for all advertising, securing meeting space, etc. Logan College of Chiropractic will make application for license renewal to the states you indicate and process attendance verifications.

At the conclusion of the seminar please forward to Logan College Postgraduate the seminar roster which should include attendee name, address, telephone number, and state license number. Also please forward the seminar sign in/out sheets. All documents must be in a typed format. This allows us to forward to the attendees their attendance verifications and report hours of attendance to state boards.
